INDICATIONS

Simicina® is a nutraceutical food supplement containing Organic Cranberry capsules in one container and D-Mannose capsules in another. Both cranberry and D-Mannose have been shown to support a healthy urinary tract.

PHARMACOLOGY/RESEARCH

Organic Cranberry:

Cranberries (and blueberries) contain a variety of different antioxidant compounds, including anthocyanins, flavonols, terpenes, hydroxycinnamic and hydroxybenzoic acids, resveratrol and some vitamins and minerals. There have been studies that have examined some of these antioxidants to see if they have an impact on E.coli.,the bacteria that causes approximately 80% urinary tract infections in the community. Cranberry does not demonstrate a bactericidal effect ( i.e does not kill bacteria directly), but appears to work by preventing the adhesion of pathogenic strains of E Coli to the surface cells of the bladder and urethra (uroepithelium) and thereby reduce colonisation and progression to invasive infection. There is in vitro evidence that some of the proanthocyanidin polymers in cranberries are responsible for this activity.

It is important to note that while these studies were promising, they were not powered to demonstrate whether cranberry could treat urinary tract infections. Other evidence of some clinical studies on cranberries and urinary tract infections from the 1960s, showed that women who drank cranberry juice had fewer bacteria in their urine and a reduction in symptoms. The study however provides limited evidence as there was no placebo or control group. A further study, published in 2001, compared recurrence of urinary tract infections in women who drank a combination of lingonberry and cranberry juice with a control group who had no intervention. The results were positive for the active group.

In a 2016 year-long study of 147 women living in nursing homes, taking two daily cranberry capsules decreased bacteria levels in their urine in the first 6 months of the study and a 2012 research review of 13 clinical trials suggested that cranberry may help reduce the risk of UTIs in certain groups, including women with recurrent UTIs, children, and people who use cranberry-containing products more than twice daily. Cranberry, however, has not been shown to be effective as a treatment for an existing UTI, though there is some evidence to imply a reduced severity of symptoms in some people taking cranberry.

In October 2017 a systematic review and meta-analyses of the effect of cranberry on reducing the risk of UTI recurrence in otherwise healthy women was published in the Journal of Nutrition (American Society of Nutrition). With a collective dataset of 1495 patients, the results showed a 26% risk reduction of recurrent UTI and the review concluded that cranberry may be effective in preventing UTI recurrence in otherwise healthy women, though larger studies would be needed to confirm findings.

D-Mannose:

Preliminary clinical trials in Europe show that D-mannose may treat or prevent urinary tract infections (UTIs). Research suggests the E Coli bacteria bind to D-mannose in preference to uroepithelial cells (surface cells of the bladder and urethra). The implication of this is that bacteria are excreted in the urine rather than sticking to uroepithelium reducing the risk of urinary tract infection.

A study published in World Journal of Urology in 2014, examined the use of D-mannose (in addition to antibiotic treatment) in 308 women with acute UTI and a history of recurrent UTIs. After one week of treatment with antibiotics, participants took prophylactic D-mannose powder, or the antibiotic nitrofurantoin, or nothing for six months. During the six-month period, the rate of recurrent UTIs was significantly higher in women who took nothing compared to those who took D-mannose or nitrofurantoin. A small pilot study in European Review for Medical and Pharmacological Sciences found that D-mannose administered twice daily for three days followed by once a day for ten days resulted in a significant improvement in symptoms, UTI resolution, and quality of life. Those who received D-mannose for six months following treatment had a lower rate of recurrence than those without supplementation.

RECOMMENDED DOSAGE

One Organic Cranberry 400mg capsule and one D-Mannose 550mg capsule taken at the same time three times a day. The capsules should be swallowed whole with a glass of water to support a healthy urinary tract.
